Insurance FAQs

  • I currently do not work with insurance. I am an out-of-network provider. I am able to help you prepare a superbill (receipt) to submit to your insurance for potential reimbursement.

Please note that in order to provide superbills, I have to assign you a diagnosis. I will always make sure to include you in the process of adding a diagnosis to your file to make sure that it is representative of your experience.

Please note that reimbursement is typically only accepted by PPO insurances.

    5. How many out of network sessions are allowed per year?

    6. Is pre-authorization needed? If so, after how many visits?

    7. What is my out-of-pocket maximum? (This is the amount you pay before insurance begins to pay 100%)

    8. What is the claims address or electronic payor ID for Mental Health Claims

    9. If seeking couples / relational sessions, ask: Are the CPT codes 90847 or 90846 (Couples and Family Therapy) covered?

  • Speak to your insurance to see their requested process. I can send you the subperbill each month or at any other frequency that works for you. Then you will submit these as claims for reimbursement.
