Insurance FAQs

I currently do not work with insurance. I am an out-of-network provider.
I can provide a superbill (receipt) to submit to your insurance for potential reimbursement. Please note that reimbursement is typically only accepted by PPO insurances.
In order to provide superbills, I have to assign you a diagnosis. I will always make sure to include you in the process of adding a diagnosis to your file to make sure that it represents your experience.

Please note that reimbursement is not guaranteed. If you have HMO coverage, Medicaid, Medi-Cal, or Medicare you likely cannot seek reimbursement.
It is recommended you use your insurance portal to check your benefits and see what the Out-Of-Network benefits are. Other resources such as Mentaya, Reimbursify, and Thrizer can check your benefits for you.
If you decide to submit superbills on your own, it is recommended that you call your insurance provider and ask the following questions:
Request to speak about Outpatient Mental Health benefits.
What is my copayment (flat fee) or coinsurance (percent)?
What is my deductible (if applicable) and how much have I met of the deductible so far?
Are my benefits in effect? When do they renew?
How many out of network sessions are allowed per year?
Is pre-authorization needed? If so, after how many visits?
What is my out-of-pocket maximum? (This is the amount you pay before insurance begins to pay 100%)
What is the claims address or electronic payor ID for Mental Health Claims
If seeking couples / relational sessions, ask: Are the CPT codes 90847 or 90846 (Couples and Family Therapy) covered?
Is my therapist’s license covered? Associate Marriage and Family Therapist, Callista Cox, National Provider Identifier (NPI): 1912654583
Is my therapist’s fee (175 for 50 minutes) within the plan’s UCR (Usual, Customary, Reasonable Fee)?

You have multiple choices in seeking reimbursement. You can outsource this through platforms like Mentaya, Reimbursify, or Thrizer. These are more hands-off options that can include refunds if your superbills are denied.
The group practice I am a part of is partnered with Mentaya. They charge a 5% fee per claim, which includes handling any paperwork, dealing with denials, and calling insurance companies. Thrizer charges $2 + 1% per claim. Reimbursify is free for first use, after that $3.99 per claim or $24.99 per 10 claims.
